• Furnish Player’s Name, Date of Shot, Type of Award, Hole Location,
Country Club and Logo. Also furnish your name and contact information.
• If the course is near enough to us we'll take photos ourselves. If not,
we'll have you send us some photos from which we'll choose the ideal view.
Please check with us to see if we already have the hole on file.
• Send photos, if necessary, and $50 deposit with all
information to be included in the award to:

• We will respond to confirm the information provided.
• Once we have final confirmation of the scene selected and information provided, we will produce and deliver your award in 4-6 weeks.
